Integrated post-acute care management services provided by WellSky®
Zing is partnering with WellSky to deliver a fully integrated, post-acute care (PAC) episode management program, inclusive of skilled nursing facilities (SNF), inpatient rehabilitation facilities (IRF), long-term acute care hospitals (LTACH), and skilled home health services, facilitating high-quality, end-to-end post-acute care management. By leveraging WellSky® PAC Advance to support care coordination, utilization management, and operational management, WellSky brings real-time visibility and better coordination across the post-acute care journey to improve member experience and outcomes.
Effective October 1, 2024, WellSky will manage post-acute care facility authorizations and home health management for all Zing members.
Effective March 1, 2025, WellSky provides a comprehensive member care management program, including ED optimization and readmission prevention.

WellSky PAC Advance provider portal
Use the provider portal to access WellSky’s utilization management system to:
- Start an authorization
- Review status of an existing authorization
- Upload clinical documentation to an existing authorization
- Submit a Notice of Medicare Non-Coverage (NOMNC) [Home health providers]
For providers not using the portal, new authorization requests, clinical documentation and NOMNC submissions can be sent via fax to the WellSky PAC Advance team:
